Medicare will provide a rebate for individual allied health services such as Isodynamic Reviver under Item 10953 for Exercise Physiologists when referred by the GP that our team will report back to after you have had your sessions.
The Medicare rebate is available for a maximum of five services per patient per calendar year although if all services are not used during the calendar year in which the patient was referred, then the unused services can be used in the next calendar year.
Unfortunately additional services under this rebate in the same calendar year are not possible in any circumstances. While the Medicare rebate covers most of the cost, there is still an out-of-pocket fee that will have to be covered. Please call our team to have this clarified before conducting your first session.
It is important to note that patients must have a GP Management Plan and Team Care Arrangements prepared by your GP, or be residents of a residential aged care facility who are managed under a multidisciplinary care plan.
The need for allied health services must be directly related to a your chronic condition and identified in your care plan. Your GP will determine whether your chronic medical condition would benefit from an allied health service such as Isodynamics Reviver, deployed by our fully-qualified and ESSA certified Exercise Physiologists.
Your GP can get the referral form they need from the Department of Health website or a form that contains all the components of the Department’s form. Note that when all referred services have been used, or a referral to a different allied health provider is required, patients need to obtain a new referral.
To be potentially eligible, you must be:
A community-based patient with a chronic (or terminal) medical condition and a GP providing the following Chronic Disease Management (CDM) services; GP Management Plan (GPMP) - (item 721) and Team Care Arrangements (TCAs) - (item 723), or
Residents of a residential aged care facility if your GP has contributed to a multidisciplinary care plan prepared for them by the residential aged care facility or to a review of the multidisciplinary care plan (item 731), or
Have a chronic medical condition that has been (or is likely to be) present for six months or longer. Note there is no list of eligible conditions, however, the CDM items are designed for patients who require a structured approach and to enable GPs to plan and coordinate the care of patients with complex conditions requiring ongoing care from a multidisciplinary team, or
A patient with complex care needs if you need ongoing care from a multidisciplinary team consisting of your GP and at least two other health or care providers.
